Position Summary:

Responsible and accountable for decisions and actions in the provision and management of Nursing care for patients in a culture of “care partnership model”

Demonstrating knowledge, experience and the ability to care for patients with the following: initiation and maintenance of IV therapy, administration of medication above the drip chamber, blood and blood product administration, central venous access devices (PICCs), blood glucose monitoring, nasogastric tubes, wound management skills, routine infection control practices, normal and abnormal blood values

Responsible for the provision of nursing care, teaching and coaching to clients and their families

Responsible and accountable for the provision of nursing care in accordance with the standards of nursing practice and Trillium Health Partners Best Practice Protocols
Qualifications:

Graduate of a recognized College or School of Nursing
-
Current certification or eligibility for certification of competence from the College of Nurses of Ontario is required

Baccalaureate degree preferred
-
Minimum of 2 years acute care medical/surgical experience required

Minimum of 5 years in an Outpatient / Ambulatory setting preffered
-
The ability to provide nursing care within a primary nursing framework.

The ability to clearly articulate nursing assessments and patients' goals of care.

The capacity to demonstrate strong organizational, prioritization and problem-solving skills
-
Current BCLS
-
Previous experience in providing and delivering patients and families with educational teaching
-
Demonstrates excellence in communication skills and interprofessional team work
-
Demonstrates strong attention to patient and family-centered care and establishing therapeutic relationships
-
A physical assessment course required
-
Evidence of on-going professional development preferred

Satisfactory attendance and performance record required
